10th-12th grade Tryouts:  November 21 & 22 from 5-7pm

Students registering for athletic activities must have a current MSHSL Sports Qualifying Physical Examination Form on file to be eligible to register and participate.

• The Sports Qualifying Physical Exam form expires 3 years from the date of the exam.
• You will not be able to register for this activity if the expiration date occurs during the season for which you are registering.
• If a current exam date appears on your Skyward Family Access Account, it is not necessary to submit a duplicate.
• Submit forms accordingly:
